As a member of the Care Management team, the Nurse Care Coordinator works collaboratively with clients to develop a Person Centered Service Plan (PCSP) based on a thorough assessment aimed at enhancing and sustaining health, mobility, and community engagement.
This role is integral to fulfilling the mission and objectives of Independence Care System.
Key Responsibilities
On a semi-annual basis, or following a significant change in condition, conduct a comprehensive in-home evaluation that reflects the client's medical history, functional abilities, and health stability—including chronic conditions, treatment protocols, current healthcare providers, and the client's prioritized concerns.
Engage with the client's healthcare providers to discuss relevant findings and facilitate the involvement of the primary physician as part of the care team.
Develop a Person Centered Service Plan (PCSP) featuring SMART interventions and objectives, based on assessment results, client preferences, and priorities, aimed at maximizing health, mobility, and community participation.
Collaborate with Care Navigators and other team members in monitoring and updating a portfolio of client care plans.
Identify and assist in the development of necessary resources and services required by clients.
Ensure thorough and timely documentation of all activities.
Respond to the needs of clients and the organization, even when those needs extend beyond the specifics of this job description.
The salary range for this position is $90,000 per year.
Qualifications
Minimum Requirements:
- Associate's Degree (if accompanied by extensive visiting nurse experience) or Bachelor's in Nursing.
- Registered Nurse currently licensed to practice in New York State.
- Minimum of 1 year of nursing experience.
- Experience in conducting and documenting assessment interviews.
- Highly self-motivated with the ability to manage time independently.
Additional Qualifications:
- Experience with Medicaid populations and/or individuals with physical disabilities is advantageous.
- Knowledge of healthcare and disability issues and systems is preferred.
- UAS experience is a plus.
- 28 days of PTO, plus 11 holidays, in your first year.
- 35-hour work week.
- An average of one day per week of remote work.
