As a member of the Care Management team, the Clinical Care Coordinator partners with clients to develop a Person Centered Service Plan (PCSP) grounded in a thorough assessment, aimed at enhancing health, mobility, and community engagement. This role is integral to fulfilling the mission and objectives of Independence Care System.

Key Responsibilities

  1. Conduct comprehensive in-home assessments semi-annually or following significant changes in health status, considering the client's medical history, functional abilities, and health stability, including chronic conditions and treatment plans.
  2. Collaborate with the client's healthcare providers to share relevant findings and ensure the primary physician is involved as part of the care team.
  3. Create a Person Centered Service Plan (PCSP) featuring SMART goals and interventions, tailored to the assessment results, client preferences, and priorities, with the aim of optimizing health, mobility, and community involvement.
  4. Work alongside Care Navigators and other team members to monitor and update a portfolio of client care plans.
  5. Identify and assist in the development of necessary resources and services for clients.
  6. Ensure thorough and timely documentation of all activities.
  7. Address the needs of clients and the organization, even when those needs extend beyond the specific duties outlined in this description.

The compensation for this role is competitive, with a salary range of $90,000 per year.

Qualifications

Minimum Requirements

  • Associate's Degree (or equivalent extensive experience in visiting nursing) or a Bachelor's Degree in Nursing.
  • Current Registered Nurse license in New York State.
  • At least one year of nursing experience.
  • Proficient in conducting and documenting assessment interviews.
  • Self-motivated with strong time management skills.

Preferred Qualifications

  • Experience working with Medicaid populations and/or individuals with physical disabilities is advantageous.
  • Familiarity with healthcare and disability systems is preferred.
  • Experience with UAS is a plus.
