Registered Nurse Care Coordinator - Primarily Remote

As a vital member of our healthcare team, the Registered Nurse Care Coordinator plays a crucial role in managing patient care effectively. Compensation:

Annual Salary: $93,000 - $116,800

Key Responsibilities

  • Oversee and coordinate benefits and services for all assigned members.
  • Conduct comprehensive care management and disease-specific evaluations.
  • Initiate timely telephonic care management outreach based on assessed risk levels.
  • Address and resolve complex issues and member grievances affecting service delivery.
  • Provide health education and resources to members and their caregivers.
  • Assess Social Determinants of Health (SDoH) and facilitate care coordination to eliminate barriers to care.
  • Identify safety concerns for members and intervene or refer to appropriate resources as needed.
  • Ensure the delivery of high-quality, cost-effective care tailored to the needs of the population served.
  • Advocate for members and caregivers to secure necessary healthcare services to enhance their quality of life.
  • Utilize the Care Management process to prioritize, plan, and implement interventions aimed at achieving self-care goals.
  • Promote adherence to treatment plans through education, coaching, and support.
  • Educate and coordinate resources to minimize unnecessary emergency room visits and hospitalizations.
  • Encourage the use of primary care, specialty services, preventive health measures, and guideline-based treatments.
  • Participate in interdisciplinary team meetings, contributing to customer service-related discussions.
  • Maintain confidentiality of member information and comply with company policies.
  • Ensure adherence to payor policies and all relevant Federal and State regulations.
  • Implement and interpret organizational policies and regulatory requirements.
Required Qualifications
  • Associate's degree in nursing or a bachelor's degree in physical therapy.
  • At least two years of experience as a registered nurse.
  • Valid license and current registration to practice as a Registered Professional Nurse or Physical Therapist in New York.
  • Population Care Coordination Certification is preferred.
  • Certifications in Care Management, Case Management, OASIS, or similar areas are advantageous.
