PRC Case Manager

St. Catherine's Center for Children is seeking a highly motivated and compassionate PRC Case Manager to join our team. As a PRC Case Manager, you will play a critical role in supporting and delivering specialized services to post-adoption, post-guardianship, and kinship caregiving families.
Job Summary:The PRC Case Manager will work in a team environment to provide community outreach, complete face-to-face contacts with families in their homes, and complete required case documentation. You will also provide 1:1 counseling sessions, participate in the design and delivery of educational events, family engagement activities, respite care, and other program-related events.
Responsibilities:- Manage a caseload of families, working to ensure the program meets all performance targets.
- Complete face-to-face contacts with families in their homes, completing all required case documentation.
- Provide 1:1 counseling sessions, incorporating trauma-informed support and motivational interviewing into the program milieu.
- Support the program's design and delivery of parent education sessions, educational events, pro-social respite activities, and family engagement.
- Assist in the delivery of presentations to community groups and local organizations in various targeted communities.
- Safely transport community members and/or their children in the delivery of program services.
- Actively communicate program information, concerns, needs, and progress to program leadership.
- Bachelor's Degree in Human Services or related field, preferred.
- In lieu of an advanced degree, related lived experience, other related professional experience, and/or educational level may be considered as qualifying for this position.
- Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges, mental health diagnoses, and/or trauma history, a plus.
- Knowledge of and ability to utilize community resources; capable of building strong working relationships with regulatory bodies.
- Experience with NYS OCFS CONNECTIONS or other electronic record system, preferred.
- Ability to handle sensitive family and individual data with strict confidentiality.
- Familiarity with kinship caregiving and wraparound services in a family-centered system of care, a plus.
- Solid written and verbal communication skills, with the ability to clearly and enthusiastically deliver the program's educational sessions for program participants.
- A current Clean & Valid New York State Driver's License (actively licensed at least one year) and reliable vehicle for transporting clients to community resources, meetings, etc.
- Proficiency in various computer applications: Microsoft Office Suite (Word, Excel, PowerPoint), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.).
- Comfortable working with and relating to culturally diverse and economically disadvantaged populations; ability to exhibit empathy for those in need, supporting their growth and service plan goal achievement.
- Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude.
- A compassionate, warm and caring mindset; capable of exhibiting empathy for those in crisis.
- Qualities of positivity, flexibility and adaptability; A professional record of integrity, proven reliability and dependability.
- Capable of exercising ethical, independent judgment and discretion, handle sensitive data with strict confidentiality.
- Ability to pass extensive pre-employment background check requirements, including fingerprinting.
