Job Title: PRC Case Manager, Delaware County

St. Catherine's Center for Children is seeking a highly motivated and compassionate PRC Case Manager to join our team in Delaware County. As a PRC Case Manager, you will play a vital role in supporting post-adoption, post-guardianship, and kinship caregiving families in our community.

Job Summary:

The PRC Case Manager will work in a team environment to provide specialized services to families, including community outreach, face-to-face contacts, case documentation, and educational events. The ideal candidate will have a strong understanding of family-centered services, trauma-informed support, and motivational interviewing.

Responsibilities:
  • Manage a caseload of families, ensuring the program meets all performance targets.
  • Complete face-to-face contacts with families in their homes, documenting all required case information.
  • Provide 1:1 counseling sessions, incorporating trauma-informed support and motivational interviewing.
  • Support the design and delivery of parent education sessions, educational events, and family engagement activities.
  • Participate in community outreach, delivering presentations to community groups and local organizations.
Requirements:
  • Bachelor's Degree in Human Services or related field, preferred.
  • Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges, mental health diagnoses, and/or trauma history.
  • Strong interpersonal skills, with patience, flexibility, and adaptability.
  • Ability to handle sensitive family and individual data with strict confidentiality.
  • Clean and valid New York State driver's license; actively licensed for at least one (1) year, with a reliable vehicle to transport program participants if needed.
