Job Summary

We are seeking a highly skilled and experienced Transplant Coordinator to join our team at Virginia Jobs. As a key member of our multidisciplinary team, you will be responsible for providing exceptional patient care and coordination of transplant services.

Key Responsibilities
  • Develop and implement individualized plans of care for transplant patients
  • Provide relationship-based care and education to patients and families
  • Collaborate with multidisciplinary team members to ensure seamless care coordination
  • Manage complex care coordination and treatment plans
  • Ensure timely communication with internal and external stakeholders
Requirements
  • Bachelor of Science in Nursing from an accredited nursing program
  • 2 years of relevant experience
  • Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia
  • American Heart Association (AHA) Health Care Provider BLS certification
Preferred Qualifications
  • Relevant specialty board certification
