Job Overview:

The WelbeHealth PACE program is designed to support seniors in maintaining their independence within their communities by offering extensive medical care and essential services.

The Registered Dietitian will play a key role in the development and execution of the WelbeHealth nutrition services program, collaborating closely with the interdisciplinary team (IDT).

In this position, you will deliver vital nutrition services, including assessments and counseling, while also acting as a liaison between WelbeHealth and food service providers. You will be responsible for menu development and ensuring compliance with all relevant regulations regarding kitchen operations and meal service.


Key Responsibilities:
1. Conduct thorough nutrition assessments to identify the needs, preferences, and goals of participants, actively engaging in IDT meetings to formulate care plans.
2. Provide and document direct nutrition interventions in alignment with physician directives and participant care plans, including the implementation and monitoring of specialized diets and nutrition counseling for participants and their families.
3. Oversee menu planning and meal delivery processes to guarantee adherence to safety and regulatory standards, ensuring participants receive appropriate diets.
4. Supervise nutrition staff, which includes hiring, training, and mentoring team members.
5. Facilitate the intake and enrollment process to ensure prospective participants navigate enrollment efficiently.

Qualifications:

- Registered Dietitian with a minimum of four (4) years of experience in directing nutrition services within a similar environment.
- Current Basic Life Support (BLS) / First Aid Certification is mandatory.
- Reliable transportation is essential.
- Strong leadership, organizational, and communication skills, particularly in settings involving seniors, their families, and IDT members.
- At least one (1) year of experience working with frail or elderly populations.
