The WelbeHealth PACE program is designed to assist seniors in maintaining their independence within their communities by offering comprehensive healthcare and community services. Our mission is to provide exceptional care and compassion to the most vulnerable senior populations through a value-based approach.
The Clinical Dietitian plays a crucial role in establishing and executing the nutrition services program at WelbeHealth, collaborating closely with the interdisciplinary team (IDT). This position involves delivering nutrition services, including assessments and counseling for participants, while also acting as a liaison between WelbeHealth and food service providers. Responsibilities include menu development and ensuring adherence to all relevant regulations regarding kitchen operations and meal service.
Key Responsibilities:
- Perform nutrition assessments to identify the needs, preferences, and goals of participants, actively engaging in IDT meetings to formulate participant care plans.
- Provide and document direct nutrition interventions as per physician directives and participant care plans, including the implementation and monitoring of specialized diets and nutrition counseling for participants and their families.
- Oversee menu planning and meal delivery to ensure compliance with safety and regulatory standards, guaranteeing that participants receive their prescribed diets.
- Lead the nutrition team, including recruitment, training, supervision, and mentorship.
- Facilitate the intake and enrollment process to ensure prospective participants navigate enrollment efficiently.
Qualifications:
- Registered Dietitian with a minimum of four (4) years of experience managing nutrition services in a comparable environment.
- Current Basic Life Support (BLS) / First Aid Certification is mandatory.
- Reliable transportation is required.
- Strong leadership, organizational, and communication skills, particularly in interactions with seniors, their families, and IDT members.
- At least one (1) year of experience working with frail or elderly populations.

The salary range for this position is $74,612 - $89,535 per year, plus bonuses and equity. WelbeHealth offers a competitive total rewards package, including a 401k match, healthcare coverage, and a variety of other benefits. Actual compensation will be adjusted based on experience and qualifications.
