Position Overview:
We are seeking a dedicated professional to join our team. This role involves providing essential nutrition services to support our mission of enhancing the quality of life for seniors in our care.

The WelbeHealth PACE program is committed to helping seniors maintain their independence by offering comprehensive medical and community-based services.

Our goal is to serve vulnerable populations with compassion and quality care through a value-driven approach.

The Clinical Dietitian Specialist will be responsible for the development and execution of the nutrition services program, collaborating closely with the interdisciplinary team (IDT).

This position includes delivering nutrition assessments and counseling to participants, acting as a liaison with food and supplement service providers, creating menus, and ensuring compliance with all kitchen and meal service regulations.


Key Responsibilities:
1. Conduct thorough nutrition assessments to identify the needs, preferences, and goals of participants, actively participating in IDT meetings to formulate care plans.
2. Deliver and document nutrition interventions as per physician orders and participant care plans, including the implementation and monitoring of specialized diets and providing nutrition counseling.
3. Oversee menu planning and meal delivery to ensure compliance with safety and regulatory standards, ensuring participants receive the appropriate diets.
4. Supervise the nutrition staff, including recruitment, training, and mentorship.
5. Facilitate the intake and enrollment process to ensure timely and effective onboarding of prospective participants.

Qualifications:

- Registered Dietitian with a minimum of four (4) years of experience in a similar role.
- Current Basic Life Support (BLS) / First Aid Certification is required.
- Reliable transportation is essential.
- Strong leadership, organizational, and communication skills, particularly in interactions with seniors, their families, and IDT members.
- At least one (1) year of experience working with elderly or frail populations.
