Key Responsibilities:- Supervise and monitor the performance of assigned home health care teams to ensure high-quality patient care and compliance with regulatory requirements.
- Develop and implement effective case management strategies to achieve optimal patient outcomes and ensure accurate evaluation and treatment plans.
- Review and provide feedback on electronically submitted documentation to ensure compliance with medical necessity, homebound status, visit utilization, and documentation of individualized patient assessment and transition planning.
- Participate in quality improvement activities to enhance agency performance and patient satisfaction.
- Serve as an educator and mentor for assigned home health care teams on home health documentation, clinical standards of care, and compliance.
- Two years of experience within a licensed and certified (Medicare) home health agency.
- Competent in Federal (Medicare) and Local standards, including Conditions of Participation and Local Coverage Determinations.
