As a PRN RN Home Health, you will provide coordinated skilled nursing care to patients of all age groups in the comfort of their own homes. You will work collaboratively with the interdisciplinary team to establish and achieve patient goals, ensuring high-quality patient care. This role requires a strong understanding of Medicare home health regulations, documentation requirements, and ICD-10 coding.
Responsibilities- Assess patients' physical, psychosocial, and environmental needs, documenting findings and communicating with the team.
- Complete OASIS assessments and visit paperwork according to agency policy, ensuring accurate and timely documentation.
- Communicate significant findings and changes to the Clinical Manager and physician, and document all interactions and interventions.
- Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
- Participate in on-call rotation and emergency call according to agency policy.
- Graduate of an accredited school of professional nursing.
- Minimum of one or more years of home health, public nursing, or acute hospital nursing experience.
- Familiarity with Medicare home health regulations, documentation requirements, ICD-10 coding, and PPS (preferred).
- Reliable transportation and proof of valid automobile liability insurance.
- Current BCLS Certification prior to providing patient care.
