Speech Language Pathologist, Home Health Per Diem

As a Home Health Speech Language Pathologist with CenterWell Home Health, you will play a vital role in helping patients manage chronic conditions or recover from injury, illness, surgery, or hospitalization.

Our care teams include nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers all working together to help patients rehabilitate, recover, and regain their independence.

Your key responsibilities will include:

  • Evaluating, directing, and providing speech/language pathology services to patients in the home or facility
  • Participating in the development and periodic review of the Plan of Treatment and Plan of Care
  • Administering and interpreting diagnostic tests and applications of therapeutic treatments, including audio logic screening
  • Observing, recording, and reporting changes in the patient's condition and response to treatment to supervisor and/or the physician
  • Providing instruction and training to patients in the use of alternative communication systems when appropriate
  • Providing counsel and instruction to patients, families, and healthcare staff
  • Maintaining and submitting documentation as required by the Company and/or facility
  • Preparing and submitting timely written reports of evaluations, visits, summaries, care plans, care coordination activities, and progress reports as required by Company policy
  • Participating in care coordination activities and discharge planning
  • Maintaining the highest standards of professional conduct in relation to information that is confidential in nature

We offer a competitive compensation package, including a pay range of $76,800 - $105,800 per year, and a comprehensive benefits program that includes paid time off, 401(k) retirement savings plan, employee assistance program, business travel, and accident insurance.
