Physical Therapist Home Visits Opportunity

As a Home Health Physical Therapist with CenterWell Home Health, you will play a vital role in delivering expert rehabilitation services to patients in the comfort of their own homes. Our team is dedicated to providing personalized care that promotes independence, mobility, and overall well-being.
Key Responsibilities- Plan and administer prescribed physical therapy treatment and training to patients with various injuries, illnesses, and functional disabilities.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and develop or pursue treatment programs.
- Develop and implement conditioning and rehabilitation programs consistent with physicians' plans of treatment and the overall goals of the patient/rehab team.
- Adjust treatment as needed to achieve maximum results and confer with physicians and clinical team members to obtain additional patient information.
- Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aide staff, supervise and evaluate their performance, and monitor the completion of documentation.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations, and report progress to patients' physicians, Clinical Manager, staff, patient, and family.
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
