Clinical Support Specialist

At Hospice of Westchester, we are seeking a skilled Clinical Support Specialist to join our team. As a key member of our clinical staff, you will provide critical support to our clinical social workers and spiritual care team in managing complex cases.
Key Responsibilities:
- Assist in resolving complex clinical situations through collaboration with clinical staff.
- Act as a resource for clinical staff, providing guidance and support as needed.
- Attend weekly team meetings to ensure seamless communication and coordination.
- Evaluate department members' clinical performance annually, using a field observation approach for SW staff.
- Participate in Performance Improvement activities to drive quality and excellence.
- Ensure compliance with policies, procedures, CHAP standards, NYS regulations, and Medicare Conditions of Participation.
Requirements:
- Master's Degree in Social Work (SW).
- Licensed by the State of New York as an LCSW.
- Knowledge of End-of-Life theory and practice.
- Five years of supervisory experience.
- Experience with Interdisciplinary Models of Care.
