As a Nurse Care Coordinator, you will assist in the planning, implementation, and coordination of patient care services. You will work closely with patients, their families, and our interdisciplinary team to ensure that each patient receives the highest quality care.

**Responsibilities**

* Assist in the development and implementation of patient care plans
* Coordinate with healthcare providers to ensure seamless care transitions
* Provide education and support to patients and their families
* Collaborate with our team to ensure that patient needs are met

**Requirements**

* Current New York State Registered Nurse License
* 2 years of experience in nursing
* 1 year of community hospice or home health experience preferred
