Nursing Care Coordinator Job Description

Graham Windham is seeking a skilled Nursing Care Coordinator to join our Care Coordination program. As a key member of our team, you will play a vital role in ensuring that health services for our children are coordinated with birth and foster parents, case planning, and healthcare provider teams.

Key Responsibilities:
  • Coordinate between Graham Windham, case planning, primary healthcare providers, and birth and foster parents on the health plan.
  • Conduct initial health and risk assessments, retrieve health history, and obtain required consent.
  • Utilize various EHR systems for scheduling, uploading, and tracking of medical information.
  • Manage appointments and provide health summaries to case planners in advance of the Family Assessment and Service Plan deadline.
  • Ensure adherence to all applicable county, state, and other regulations.
  • Participate in High-Risk Meetings and ensure that the best provision of services is offered for "high-risk" kids on caseload.
Requirements:
  • Licensed Practical Nurse (LPN) or higher required.
  • Experience and skill in providing care coordination, preferably with children and families.
  • Microsoft Office and familiarity with NYS Connections system are a plus, as are other electronic health records systems.
  • Excellent communication, time management, and organizational skills.
  • Bilingual Spanish/English preferred.
Benefits:
  • Base Salary or Hourly Rate: $57,275.00/year ($31.47/hour)
  • FLSA Status: Non-exempt
  • Benefits Eligibility: This position is benefits eligible; sample benefits include:

    • Potential to participate in our Performance Based Merit Award program at the end of each fiscal year.
