Job Title: Registered Nurse - Care Coordinator

Job Summary:

We are seeking a highly skilled Registered Nurse to join our team as a Care Coordinator. The successful candidate will be responsible for providing high-quality nursing care to individuals with complex needs, while also coordinating their care with other healthcare professionals.

Key Responsibilities:

  • Assess and Evaluate: Conduct thorough assessments and evaluations of individuals' nursing needs, identifying areas for improvement and developing personalized care plans.
  • Coordinate Care: Collaborate with healthcare professionals, families, and caregivers to ensure seamless coordination of care, including medication management, wound care, and other medical needs.
  • Develop and Implement: Develop and implement individualized care plans, taking into account the unique needs and goals of each individual.
  • Monitor and Evaluate: Continuously monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Communicate Effectively: Communicate effectively with individuals, families, and healthcare professionals to ensure that care needs are met and that individuals receive the support they require.
  • Document and Record: Accurately document and record all aspects of care, including assessments, care plans, and progress notes.
  • Participate in Interdisciplinary Teams: Participate in interdisciplinary teams to discuss individual care plans, share knowledge, and coordinate care.
  • Stay Up-to-Date: Stay up-to-date with the latest research, best practices, and regulatory requirements to ensure that care is evidence-based and compliant.

Requirements:

  • Licensure: Current licensure as a Registered Nurse in the state of New York.
  • Experience: At least 2 years of experience in a healthcare setting, preferably in a care coordination or case management role.
  • Education: Bachelor's degree in Nursing or related field.
  • Certifications: Current certifications in CPR and First Aid, with the ability to obtain other relevant certifications as required.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with individuals, families, and healthcare professionals.
  • Computer Skills: Proficiency in electronic medical records and other relevant software systems.
