Job Overview

We are seeking an experienced Patient Care Coordinator to join our team at Rehabilitation Care Coordination. As a key member of our team, you will be responsible for providing exceptional patient care and support to individuals with catastrophic injuries and/or mental illnesses.

This is a part-time per diem position that requires independent work and effective communication skills. You will need to drive to appointments and for patient visits as needed. A California Nursing License and Bachelor's degree in Nursing are required.

The ideal candidate will have at least 3 years of acute nursing experience, with rehabilitation experience preferred. Fluency in languages besides English is a plus. You will be a self-starter with excellent written and verbal communication skills and able to work effectively independently and as part of a team.

In this role, you will assess, plan, implement, coordinate, and evaluate patient care needs. You will also coordinate resources and benefits available through various government, community, and local educational institutions.
