Patient Care Coordinator

We are seeking an experienced and compassionate individual to join our team as a Patient Care Coordinator. Key responsibilities include:
- Providing basic care to patients under the direct supervision of registered nurses
- Assisting patients with activities of daily living, such as bathing, dressing, and feeding
- Notifying nursing staff of any changes in patient condition, including vital signs, level of consciousness, and pain management
- Contributing to a positive and supportive team environment
To be successful in this role, you must have:
- Graduated from an accredited Nursing Assistant program or demonstrated equivalency
- Completed 2nd semester of an accredited Nursing Program or hold a valid HHA certification with 1-2 years' experience
- Current Basic Life Support (BLS) certification for Healthcare Providers from the American Heart Association
- Strong communication and interpersonal skills, with the ability to manage multiple tasks and priorities
