We are currently seeking a dedicated Clinical Services Coordinator to join our ECM team. This critical role involves coordinating all aspects of ECM for our members, working closely with other care managers and healthcare providers to ensure seamless and effective care management and service delivery.

Job Summary:

This is an exciting opportunity to make a meaningful difference in the lives of our members. As a Clinical Services Coordinator, you will be responsible for developing, implementing, and regularly reviewing care plans tailored to meet the specific needs of each member.

Responsibilities:
  • Care Plan Development and Implementation: Create and manage care plans that address the unique needs of each member.
  • Collaboration and Teamwork: Work closely with other care managers and healthcare providers to ensure comprehensive care and promote a collaborative approach.
  • Education and Support: Engage with members and their families to provide education and support for better health outcomes.
Requirements:
  • Qualifications and Experience: Significant experience in care management, preferably in a community-based setting, along with strong interpersonal and communication skills.
  • Candidate Requirements: Ability to work effectively as part of a multi-disciplinary team, knowledge of healthcare systems and regulations, particularly Medi-Cal benefits and ECM requirements, and empathy and commitment to person-centered care.
