About the Role

We are seeking a highly skilled Care Coordination Specialist to join our team at Kaiser Permanente in Riverside, CA. As a key member of our Utilization Management department, you will play a critical role in ensuring that patients receive the right care at the right time.

In this part-time position, you will work closely with our interdisciplinary team to provide care coordination and support for patients under the direction of a Registered Nurse. Your primary responsibilities will include:

  • Conducting medical necessity screening and reviewing patient charts to ensure compliance with evidence-based practice and regulatory requirements.
  • Collaborating with the healthcare team to develop and implement effective transition plans for patients transitioning to post-acute care.
  • Communicating with patients, families, and caregivers to ensure they understand their treatment plans and options.

To be successful in this role, you must have excellent organizational skills, strong verbal and written communication skills, and the ability to work effectively in a fast-paced environment.
