Job Title: Nurse Care Coordinator I Job Specialty: Home Health Job Duration: 13 weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: No Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience License: Must have a valid nursing license in hand Certifications: CPR, BLS Must-Have: - Experience with HomeCare HomeBase (HCHB) EMR and proficiency in OASIS within the last year- Reliable transportation, a current driver's license, and required liability insurance- Willingness to travel up to 75 miles from the home clinic- Open to on-call rotations and weekend work as required Job Responsibilities:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and participate in periodic office meetings.
- Manage a caseload of 5-8 patients per day.
- Complete documentation within 24 hours of patient appointments.
- Achieve weekly productivity expectations of 30 points.
- Be open to floating to nearby clinics if patient census is low.
- Participate in weekly clinic meetings.
- Maintain complete and timely medical records for each patient.
- Efficiently plan travel routes to optimize time and minimize travel between patient homes.
