Key Responsibilities

  • Ensure timely scheduling and documentation of patient/client appointments.
  • Engage in coordinating care with management and the interdisciplinary team.
  • Communicate effectively with patients/clients, team members, and other healthcare professionals.
  • Process reports promptly to maintain compliance.
  • Address agency and patient/client needs with professionalism and creativity.
  • Uphold the agency's philosophy and mission by presenting a positive image to all stakeholders.
  • Demonstrate commitment to professional growth and competency.
  • Maintain confidentiality while providing effective communication to patients/families and team members.
  • Perform additional duties as assigned.
  • Adapt to a routine office environment with a flexible schedule.
  • Occasionally lift, pull, carry, and push items weighing up to fifty pounds.
  • Work under some stressful conditions to meet deadlines and patient/client needs.
  • Utilize problem-solving skills as required.
Qualifications

  • High School diploma or equivalent.
  • Preferred: One year of experience in a scheduling role within home health.
  • Exceptional communication skills and experience with electronic medical records.
  • Ability to multitask in a dynamic environment.
  • Strong organizational and customer service skills.
  • Professional demeanor and phone etiquette.
