Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our HouseCalls team in Yamhill County, OR. As a member of our integrated medical team, you will provide annual clinical assessments to patients in the comfort of their homes, helping to identify and reduce health risks and coordinate appropriate follow-up care.
Key Responsibilities- Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as appropriate.
- Identify diagnoses to inform care management and active medical management.
- Communicate findings to inform members' primary care physicians of potential gaps in care.
- Intervene in urgent and emergent situations.
- Educate members on disease processes, medication, and compliance.
- Active NP or PA license or ability to obtain by start date.
- Active national NP or PA certification or ability to obtain national certification in the state of assignment by start date.
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and various dwelling conditions.
- Reliable transportation for home visit assessments.
- May be requested to obtain additional licensure in other geographic areas.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
