Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment provider. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, follow-up, and clear documentation according to Matrix guidelines and protocols.

Responsibilities
  • Conduct comprehensive health assessments, medical history, and diagnosis in a variety of settings, including home visits, skilled facilities, and telehealth visits.
  • Collaborate with physicians, case managers, social workers, family members, and other healthcare professionals to provide patient-centered care.
  • Develop and implement individualized care plans, including health education, physician referrals, and case management referrals.
  • Provide follow-up care and documentation to ensure continuity of care.
Requirements
  • Master's degree in Nursing or Physician Assistant studies, or commensurate experience and satisfactory completion of NP or PA licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
  • Board certification by the NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant, with the ability to travel in-state or out-of-state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills, with a positive and outgoing personality.
