Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a PRN/Part Time Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to improve health outcomes.
Responsibilities- Conduct thorough health assessments in patients' homes, including medical history, physical examination, and laboratory testing
- Develop and implement personalized care plans to address identified health risks and improve health outcomes
- Collaborate with healthcare providers, patients, and caregivers to ensure seamless care coordination
- Provide education and support to patients and caregivers on healthy lifestyle choices and disease management
- Document all assessments and care plans in our electronic medical record system
- Masters degree in Nursing or Physician Assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patients' homes and work in a fast-paced environment
