Clinical Care Specialist

Job Description:
As a Clinical Care Specialist at Baptist Health, you will play a vital role in promoting high-quality patient care through clinical care, consultation, and patient education. Your expertise in wound, skin, and continence care will be integral in delivering individualized care to patients with complex needs.
Working closely with the healthcare team, you will coordinate patient care plans, ensuring seamless communication and collaboration. Your passion for education and patient advocacy will be essential in supporting patients and their families throughout their healthcare journey.
Requirements:
- Registered Nurse with a B.S. degree
- Kentucky License
- Certified Wound Ostomy & Continence Nurse (WOCN) certification
- Minimum of two years of nursing experience
- Experience in education preferred
