Aveanna Healthcare Job Description

We are seeking a skilled and compassionate Private Duty Nurse LVN to join our team at Aveanna Healthcare. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, helping them to recover from illness or injury and regain their independence.

Responsibilities:
  • Implement and manage patient care plans in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Assess patient needs and develop individualized care plans to meet those needs.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and care provided in a timely and accurate manner.
Requirements:
  • Current, unencumbered LPN/LVN license in the state of practice.
  • Minimum 6 months of experience as a Licensed Practical Nurse.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift 50 pounds and stand for long periods.
