As a Care Coordination Specialist, you will play a vital role in ensuring that our patients receive the best possible care. You will work closely with our healthcare team to develop and implement individualized treatment plans that meet the unique needs of each patient.

The ideal candidate for this position will possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will be responsible for coordinating patient care, managing clinical documentation, and collaborating with other healthcare professionals to ensure seamless transitions of care.

We offer a competitive salary of $63,000 - $80,000 per year, depending on experience. In addition to your salary, you will also enjoy a comprehensive benefits package, including medical, dental, and vision coverage, as well as 401(k) matching and paid time off.

About This Opportunity:

  • Support the 25-bed Facility
  • Swing Bed Level Offered, Skilled Care and Rehab Focus
  • Average of 12 to 15 Patients Per Day; with Winter months towards 25
  • Strong Team Oriented Unit; opportunity to support patients at the most critical time in a fast-paced environment

What You Will Do:

  • Provide program orientation to patients/families/caregivers
  • Act as the coordinator of patient/family/caregiver education
  • Ensure the implementation of the patients treatment plan
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments

Required Skills and Qualifications:

  • Registered Nurse
  • Associate Degree, or Bachelors Degree
