Our branch offices are located in Germantown, Fond du Lac, Waukesha, and Appleton.

We are currently seeking a Full-Time Hospice RN Case Manager for our Brown and Outagamie County territories. This role offers an exciting opportunity to work with patients and families in their homes, providing compassionate and skilled care.

Job Description: As a Hospice RN Case Manager, you will be responsible for coordinating patient care, collaborating with healthcare teams, and ensuring seamless transitions between care settings. Your expertise and compassion will make a meaningful difference in the lives of our patients and their loved ones.

Requirements: To succeed in this role, you must have:
  • A current RN licensure in the State of Wisconsin
  • 2 years of RN level nursing experience preferred
  • Excellent communication skills with patients, families, facility staff, doctors, and teammates
  • Ability to work independently
  • Good organization and effective time management skills
  • Willingness to travel to meet patient needs
  • Compliance with documentation standards and regulations
  • Prior home health experience preferred, but not required
