About the Role:
  • Identify patients with high-risk needs and provide comprehensive social work services, including behavior health and substance abuse support, to promote optimal patient outcomes.
  • Conduct thorough assessments of patients' psychological, social, environmental, and financial status, and develop personalized plans to meet their unique needs and goals.
  • Collaborate with patients, families, and community support services to coordinate and advocate for community-based services, ensuring seamless transitions and continuity of care.
Requirements:
  • Masters Degree in Social Work from a professional school accredited by the Council on Social Work.
  • Minimum 3 years of current relevant social work experience, with a proven track record of delivering high-quality patient care.
  • Substance Abuse Specialty preferred, with knowledge of evidence-based practices and interventions.
  • Knowledge of telehealth technology desired, with experience in remote patient engagement and care coordination.
  • Bilingual English/Spanish a plus, with the ability to communicate effectively with diverse patient populations.
Preferred Qualifications:
  • LCSW (Licensed Clinical Social Worker) certification, with a strong commitment to ongoing professional development and continuing education.
