Key Responsibilities:

  • Conduct thorough assessments, diagnose, and manage acute and chronic conditions for patients in their homes.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Perform routine health screenings, preventive care, and wellness visits.
  • Educate patients and their families on health management, disease prevention, and healthy lifestyle choices.
  • Provide guidance on medication management and adherence to treatment plans.
  • Address patient concerns and answer questions regarding their health and care.
  • Work closely with a multidisciplinary team, including nurses, social workers, and other healthcare providers, to ensure comprehensive patient care.
  • Coordinate care with other healthcare professionals and community resources as needed.
  • Participate in case conferences and team meetings to discuss patient care plans and progress.
  • Maintain accurate and detailed patient records in accordance with legal and regulatory requirements.
  • Ensure compliance with organizational policies and procedures.
  • Participate in quality improvement initiatives and clinical audits.

Requirements:

  • Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
  • Current and valid medical license in NJ.
  • Board certification or eligibility in Internal Medicine or Family Medicine. Experience in home-based primary care is a plus.
  • Excellent clinical skills in primary care and chronic disease management.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Please note that the annual base salary range is $180k to $225k, along with a bonus and equity ownership. Actual compensation offered to the successful candidate may vary based upon geographic location, work experience, education, and/or skill level. Details about eligibility for bonus compensation will be finalized at the time of offer.
