Job Summary

We are seeking a highly organized and detail-oriented Patient Care Coordinator to join our team at Amedisys. As a Patient Care Coordinator, you will play a critical role in ensuring the smooth operation of our healthcare services.

Key Responsibilities
  1. Scheduling and Coordination: Ensure timely scheduling of patient visits, including Oasis visits, and coordinate with field staff, patients, and Clinical Managers to maximize care center capacity and utilization guidelines.
  2. Weekly Scheduling: Prepare weekly clinician schedules and review daily or more frequently to determine capacity opportunities and ensure productivity standards are being met.
  3. Geographic Optimization: Ensure clinicians are assigned and scheduled in the most efficient geography, maximizing clinician efficiency, utilizing the lowest possible discipline, matching skill with required care, and optimizing clinician capacity.
  4. Missed Visits: Ensure missed visits are scheduled by the end of the current week or referred to the Clinical Manager for resolution.
  5. Communication: Manage calls from patients and field staff related to scheduling issues and refer clinical and performance-related issues to the Clinical Manager.
  6. Hospitalized Patients: Monitor hospitalized patients, communicate, and ensure the team follows up as necessary.
  7. Schedule Management: Complete requested schedules ensuring all orders and services (e.g., lab draw, supervisory visits, add-on visits or disciplines, etc.) are included as required.
  8. Collaboration: Support Clinical Managers and work collaboratively to ensure timely communication with patients, clinicians, referral sources, BD team, and other office personnel.
  9. Reporting and Workflow: Run all applicable reports and respond to workflow, taking appropriate actions.
  10. Patient Transfer: Assist with internal or external transfer of patients between care centers and/or Hospice services.
  11. Supply Management: May be responsible for maintaining supply closets with routine supplies, ensuring supplies are within expiration dates and packaged appropriately, and serving as a liaison with the field for patient supply needs.
  12. Referral and Intake: In the absence of an Intake Coordinator, may be responsible for the referral and intake process within the care center.
  13. Verbal Orders: If a licensed clinician, may obtain verbal orders for patients and document as applicable.
Qualifications
  1. Licensure: Current, unencumbered license to practice as a Licensed Practical Nurse/Licensed Vocational Nurse or Registered Nurse specific to that state the employee is assigned to work by the company.
  2. Education: High School diploma or GED equivalent.
  3. Experience: One (1+) year of administrative experience in a healthcare environment.
