We are seeking a highly skilled and compassionate Dialysis Coordinator I to join our Patient Pathways team at DaVita. As a key member of our team, you will play a vital role in facilitating patient education, processing timely dialysis placement referrals, and building relationships with local hospitals, doctors, and other kidney care providers.
Key Responsibilities- Facilitate patient education and provide support to patients and their families
- Process timely dialysis placement referrals and coordinate with healthcare providers
- Build and maintain relationships with local hospitals, doctors, and other kidney care providers
- Analyze and share best practices with other members of the department to improve the patient education process
- High School Diploma or equivalent required; Bachelor's Degree strongly preferred
- Case Management experience strongly preferred
- Knowledge of Medical Insurance (Medicare/Medicaid/Commercial) highly preferred but not required
- Experience in patient education highly preferred
- Experience with hospital settings highly preferred but not required
- Dialysis experience preferred; not required
- Strong customer service skills in face-to-face interaction settings
- Strong work ethic and desire to serve our patients
- Valid driver's license and ability to travel locally
