About the Role

We are seeking a highly skilled and compassionate Dialysis Coordinator I to join our Patient Pathways team at DaVita. As a key member of our team, you will play a vital role in facilitating patient education, processing timely dialysis placement referrals, and building relationships with local hospitals, doctors, and other kidney care providers.

Key Responsibilities
  • Facilitate patient education and provide support to patients and their families
  • Process timely dialysis placement referrals and coordinate with healthcare providers
  • Build and maintain relationships with local hospitals, doctors, and other kidney care providers
  • Analyze and share best practices with other members of the department to improve the patient education process
Requirements
  • High School Diploma or equivalent required; Bachelor's Degree strongly preferred
  • Case Management experience strongly preferred
  • Knowledge of Medical Insurance (Medicare/Medicaid/Commercial) highly preferred but not required
  • Experience in patient education highly preferred
  • Experience with hospital settings highly preferred but not required
  • Dialysis experience preferred; not required
  • Strong customer service skills in face-to-face interaction settings
  • Strong work ethic and desire to serve our patients
  • Valid driver's license and ability to travel locally
