Position Overview:

As a Registered Nurse (RN) Case Manager, you will be a vital part of the healthcare team at Cleveland Clinic's Hillcrest Hospital, renowned for its advanced research, cutting-edge technology, and exceptional patient care.

Key Responsibilities:
In this role, you will:

  • Work closely with inter-professional teams to ensure seamless transitional care coordination.
  • Advocate for patients, conducting holistic assessments and developing comprehensive care plans.
  • Empower patients and their families to manage their health effectively.
  • Ensure compliance with accreditation standards and state regulations.
  • Facilitate effective communication during patient handoffs to maintain continuity of care.
  • Identify and address barriers to patient transitions, proactively working to mitigate risks.
  • Engage in patient rounding, huddles, and committee meetings to enhance care delivery.
  • Complete annual competency training to maintain high standards of practice.

Qualifications:
The ideal candidate will possess:

  • A degree from an accredited nursing program.
  • Current state licensure as a Registered Nurse (RN).
  • Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • A minimum of three years of recent clinical nursing experience.

Preferred Qualifications:
Additional qualifications that will enhance your candidacy include:

  • A Bachelor of Science in Nursing (BSN).
  • Certification in care management (CTM, ACM, CCM, or AHA) within one year of eligibility.
  • Two years of experience in utilization review, quality management, or care coordination.
  • Knowledge of acute care and accreditation standards.
  • Experience in discharge planning and quality improvement initiatives.

Physical Requirements:
This position requires:

  • A full range of motion and manual dexterity.
  • The ability to stand and walk for extended periods.
  • Normal or corrected hearing and vision.
  • Occasional lifting of up to 10 pounds and sedentary work involving sitting for prolonged periods.

Adherence to Standard Precautions and use of personal protective equipment as necessary.
