Responsibilities

The RN Case Manager is responsible for delivering intermittent skilled nursing services, ensuring effective communication regarding patient progress with interdisciplinary teams, and overseeing the hospice aide staff in providing personal care. As an RN Case Manager, your duties will include:
  • Admitting patients for hospice services under physician orders
  • Conducting assessments to evaluate patient needs and collaboratively setting goals with patients
  • Reporting patient conditions and requirements for additional services to clinical leadership and physicians
  • Regularly updating care plans and addressing patient issues and goals as necessary
  • Completing informational visits and securing patient consents for hospice admission according to established procedures
  • Ensuring the implementation of the 4Ms (What Matters, Medications, Mentation, and Mobility) to provide Age-Friendly Care
Qualifications
  • Must possess a current, unencumbered registered nurse license in the state of practice or comply with Nurse Licensure Compact (NLC) regulations
  • Valid driver's license and a clean driving record are required
  • Ability to work in a field environment and demonstrate sound nursing judgment
  • Capability to assess patient needs and develop tailored care plans accordingly
