Key Responsibilities

The RN Case Manager is responsible for delivering intermittent skilled nursing services, effectively communicating patient progress with other healthcare professionals, and overseeing the hospice aide staff in providing personal care to patients. As an RN Case Manager, your duties will include:
  • Admitting patients eligible for hospice services under physician orders
  • Assessing and evaluating patient needs, identifying mutually agreed-upon goals with patients
  • Reporting patient status and needs to clinical leadership, attending physician, and hospice physician
  • Continuously updating care plans; revising and addressing patient issues and goals as necessary
  • Conducting informational visits and securing patient consents for hospice admission as per established procedures
  • Ensuring the application of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) while providing Age-Friendly Care
Qualifications
  • Must possess a current, unencumbered registered nurse license in the state of practice or comply with Nurse Licensure Compact (NLC) regulations
  • Valid driver's license and a clean driving record are required
  • Ability to work in a field setting and demonstrate sound nursing judgment
  • Capability to assess patient needs and develop individualized care plans accordingly
