Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our ACT Team at WellLife Network. As a key member of our clinical team, you will play a vital role in providing high-quality care to individuals with severe mental illnesses.

Key Responsibilities
  • Assess and evaluate the needs of assigned consumers for treatment
  • Supervise and render general nursing care, including administering injections and medications
  • Provide psychiatric and medical assessment and treatment, and develop, maintain, and supervise medication and psychiatric treatment plans
  • Collaborate with the treatment team in comprehensive assessment of psychiatric history, mental status, and diagnosis
  • Perform individual, group, and family supportive therapy and illness education
  • Transcribe prescriber's medication orders, administer prescribed injections and medications, and monitor and draw labs as needed
  • Advise prescribers of medication errors and suggest changes
  • Record vital statistics and other pertinent data on consumers, and maintain records of same
  • Liaison with physician, dietitians, hospitals, pharmacies, and consumer's family on all health situations
  • Utilize agency vehicle to conduct visits, escort consumers to appointments, transport food or goods for consumers' home or program-sponsored activity
  • Act as a clinical liaison between the team and outside entities
  • Document any progress notes in EHR within 48 hours of all client and collateral contacts
  • Ensure all documentation is completed in a timely fashion and remain in compliance with agency, OMH, and AOT regulations
Requirements
  • Bachelor's degree in nursing, registered, and at least one year of experience working with the mentally ill
  • Clinical supervisory experience is desirable
  • A clear understanding of the characteristics and problems of adults with severe mental illnesses and be knowledgeable about the ACT model
  • Computer proficiency, including the ability to use email, navigate and enter all documentation into electronic medical record
  • Strong interpersonal and communication skills and knowledge of group dynamics required
  • Valid New York State driver's license and vehicle required
