True Care NY is seeking a dedicated and detail-oriented Patient Care Coordinator to join our growing team. In this role, you will play a vital part in ensuring smooth patient onboarding and facilitating access to exceptional home care services.

As a Patient Care Coordinator, your responsibilities will encompass:

  • Gathering comprehensive patient documentation and insurance information to facilitate service authorization.
  • Thoroughly evaluating insurance coverage types and levels to determine patient eligibility for home care programs.
  • Collaborating effectively with medical professionals and insurance representatives to expedite the patient eligibility process.
  • Analyzing weekly status reports from agency contacts and completing all necessary paperwork accurately.
  • Maintaining meticulous data entry to ensure accurate reporting and outcomes, while scheduling assessments and reassessments for applicants and enrolled patients using HHA eXchange.
  • Fostering clear and timely communication with the Intake Manager, team members, and the marketing department.
  • Facilitating a seamless exchange of information between departments to schedule assessments and gather essential clinical and demographic data from patients and relevant sources.
  • Cultivating strong relationships with all agency contacts.
  • Creating, obtaining, and meticulously tracking communication notes for prior and current authorizations.

Qualifications:

  • High School Diploma or GED equivalent.
  • Fluency in Mandarin and Cantonese or Fuzhounese.
  • Experience working with database systems, a strong aptitude for retaining large amounts of information, and familiarity with HHA eXchange is highly desirable.
  • Proven track record of exceptional customer service.
  • Excellent verbal and written communication skills.
  • A personable and professional demeanor, capable of assisting patients with inquiries regarding CDPAP programs.
