Position Overview:

The Patient Care Scheduling Specialist is essential in orchestrating comprehensive patient care while serving as a liaison among the agency, patients, and various service providers. Effective communication among all parties involved and adherence to company protocols are paramount.

Primary Duties:
  • Serve as the main point of contact between caregivers and patients to guarantee ongoing, top-notch care.
  • Develop and maintain relationships with service providers to enhance patient engagement.
  • Ensure adherence to Value-Based Purchasing (VBP) measures and quality benchmarks.
  • Strategically arrange services for clients and caregivers.
  • Promote clear communication within the department concerning scheduling, attendance, and other critical matters.
  • Confirm daily caregiver attendance and prioritize staffing assignments based on patient requirements.
  • Keep all stakeholders updated on any patient care concerns and maintain accurate documentation.
  • Address inquiries from healthcare professionals, contracts, and clients promptly and courteously.
  • Uphold patient confidentiality and respect throughout all interactions.
  • Efficiently manage non-clinical inquiries from patients and caregivers.
  • Document patient care activities accurately in accordance with departmental procedures.
Qualifications:
  • Proficiency in English, Korean, and either Cantonese or Mandarin.
  • Strong verbal and written communication skills.
  • Outstanding interpersonal and customer service skills, particularly with elderly clients.
  • Adept at handling challenging situations with compassion and professionalism.
  • Ability to thrive in a dynamic and occasionally high-pressure environment.
  • Experience with the HHA Exchange platform is a plus.
