Key Responsibilities- Provide visiting home health services to an assigned caseload, including admissions, re-certifications, and discharges.
- Develop and implement individualized care plans in coordination with healthcare professionals.
- Conduct comprehensive assessments and document patient information using a tablet within the required timeframe.
- Ensure compliance with Department of Health and government regulations.
- Collaborate with the clinical manager to ensure seamless care delivery.
- Proven experience as a registered nurse.
- Valid nursing license.
- Pediatric experience.
- Must have a vehicle and be able to drive.
- Must have experience completing comprehensive OASIS assessments.
- Experience in acute care in a hospital setting or home care experience.
- Experience using Homecare Homebase (HCHB) is a plus but not required.
