HarmonyCares is a leading provider of home-based primary care services, and we are seeking a skilled Patient Care Coordinator to join our team.

About the Role

The Patient Care Coordinator will provide clinical support to HarmonyCares Medical Group patients primarily through phone encounters. They will answer inbound inquiries from patients, triaging and communicating to providers with the appropriate urgency. They will also communicate clinical information to patients on behalf of the provider.

Responsibilities
  • Provide medical triage for inbound clinical inquiries, including new/emerging medical conditions, sick calls, and medication requests.
  • Maintain a paneled caseload of lower acuity patients and perform tasks such as care plans and proactive outreach.
  • Initiate the SBAR process and utilize triage protocols when assessing a clinical situation and providing recommendations to patients and providers.
  • Document all phone calls and patient interactions in the EHR promptly, utilizing standard clinical documentation guidelines.
  • Review, transcribe, communicate, and process providers' verbal orders as requested.
  • Assist with patients' medication refill requests, including checking for duplicate requests and preparing necessary information for provider review.
Requirements
  • Ability to react decisively and quickly in emergencies.
  • Knowledge of medications, effects, indications, and contraindications.
  • Experience with multi-tasking and electronic health records (EHR) systems.
  • Ability to communicate effectively with patients, family members, and facility staff.
  • Knowledge of basic principles, practices, and techniques in primary healthcare.
Preferred Qualifications
  • Active LPN/LVN license in the state where patients are located.
  • 2 years of experience as an LPN or LVN.
  • Familiarity with Medicare and Medicaid insurances and programs.
