The Bureau of Public Health Clinics (BPHC) is seeking a highly motivated and experienced Public Health Navigator to join our team. As a Public Health Navigator, you will play a critical role in promoting a healthy community by providing New Yorkers with the resources needed to make informed and empowered health decisions.
Key Responsibilities:- Oversee caseload of STD clinic patients who are seeking HIV prevention services, ensuring that patients are connected to prevention and treatment services and adhere to established plans of care.
- Deliver health education and health promotion messages according to STD clinical protocols.
- Coordinate logistics for plan adherence, including appointment reminders, scheduling, medication adherence, arranging transportation, and other necessary arrangements.
- Collaborate with STD clinic staff to identify qualified candidates for programs and services.
- Perform rapid HIV screening and provide results and advising in a confidential manner per bureau protocol.
- Maintain liaison with healthcare providers and other agencies to expedite efficient follow-up and treatment of patients enrolled in programs.
- A baccalaureate degree from an accredited college, including or supplemented by twelve semester credits in health education, or in health, social, or biological sciences.
- Or a baccalaureate degree from an accredited college, and six months of full-time satisfactory experience in a health promotion or disease intervention/prevention program, performing one or more of the following: interviewing, conducting field investigations, assessing health risks, making referrals, or collecting and analyzing epidemiological data.
- Or a four-year high school diploma or its educational equivalent, and four years of full-time satisfactory experience as described above.
