Public Health Nurse, Bureau of Public Health Clinics

We are seeking a highly skilled Public Health Nurse to join our team at the Bureau of Public Health Clinics. As a key member of our team, you will be responsible for providing medical case management, health counseling, and referrals to clients in need of disease control and prevention services.
Key Responsibilities:
- Conduct home visits to assess clients' health status and provide guidance on treatment management and referrals.
- Interview and triage clients, applying infectious disease control guidelines and quality assurance policies.
- Perform screening tests, review results, and make referrals as needed.
- Administer medications as prescribed by a physician.
- Participate in case management meetings and record reviews.
Requirements:
- Valid New York State license as a Registered Nurse.
