Job Title: Behavioral Health Provider

At Axess Family Services, we are seeking a highly skilled Behavioral Health Provider to join our team. As a Behavioral Health Provider, you will play a critical role in providing psychosocial assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders, including chemical dependency.

Key Responsibilities:
  • Develop and implement effective treatment plans to assist clients in achieving their goals.
  • Provide comprehensive counseling services to patients enrolled in medication-assisted treatment (MAT) programs, working collaboratively with the primary care team to manage and treat chemical dependency.
  • Monitor the site's behavioral health program, identifying areas for improvement and making recommendations to enhance patient services.
  • Facilitate outreach and involvement with community-based programs and agencies to support clients' needs.
  • Provide consultation to other healthcare professionals to ensure coordination of care and achievement of desired treatment goals.
Requirements:
  • Master's degree in Behavioral Health, Social Work, Counseling, or a related field with the capacity to bill for encounters.
  • Working knowledge of behavioral medicine and evidence-based treatments for medical and mental health conditions.
  • Ability to work effectively with diverse clientele, Agency staff, and outside contacts.
  • Licensed in the State of Ohio as a Licensed Independent Social Worker (LISW)/Licensed Social Worker (LSW) or equivalent.
