Role Overview: CNY Family Care is seeking a dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to support patient triage and administrative functions within our busy practice. Following a three-month on-site training period, this role will transition to a remote work environment.
Key Responsibilities:
- Address patient inquiries and medical advice requests through the patient portal.
- Manage patient calls for medical advice and triage effectively, ensuring timely communication.
- Review and respond to various patient documents, including surgical reports, imaging results, and immunization records.
- Update patient records in the Electronic Medical Record system based on document reviews.
- Communicate significant health status changes to patients' Primary Care Physicians.
Qualifications:
Applicants must possess a valid NYS Nurse Practitioner or Physician Assistant license, with a minimum of two years of experience in a family practice or urgent care setting. Familiarity with Medent (EMR) is preferred. Candidates should demonstrate energy, a strong understanding of chronic disease management, and critical thinking abilities.
