Job Title: Clinical Care Transition Specialist

Job Summary:

The Clinical Care Transition Specialist is a key member of our team, responsible for ensuring seamless transitions of care for patients from hospital to home or alternative treatment sites. This role requires a strong understanding of infusion services and the ability to communicate effectively with patients, families, and healthcare professionals.

Key Responsibilities:

  • Proactively maintain and grow relationships with referral sources to increase sales and patient starts.
  • Provide education and assistance to patients, families, and facility staff about Option Care services and products.
  • Conduct assessments of patients to ensure they are viable candidates for alternative site care.
  • Use sales techniques to educate patients and their caregivers about the services and products provided by Option Care.
  • Effectively communicate with agency staff, medical teams, patients, and families throughout the discharge process to implement an effective treatment plan.
  • Collect, review, and complete pre-admission information and secure related signoff.
  • Partner with Account Manager and Regional Sales Director to create and execute area business and growth plans.
  • Communicate frequently with Account Manager to discuss opportunities, assess progress, and provide feedback related to promoting the services of Option Care.
  • Participate in data collection required for therapy start and patient tracking process.
  • Maintain confidentiality of patient and proprietary information and observe legal guidelines for safeguarding the confidentiality of patient and proprietary Option Care information.

Requirements:

  • Licensed Registered Nurse (RN) or licensed Pharmacist in the state of practice and at least 2 years of experience in the healthcare industry.
  • Experience establishing and maintaining relationships with individuals at all levels of the organization in the business community and with vendors.
  • Experience providing customer service to internal and external customers, including meeting quality standards of services, and evaluation of customer satisfaction.
  • Basic PC skills, including the ability to competently use internet, email, Microsoft Word, Microsoft Excel, and Microsoft PowerPoint.
  • Access to a reliable means of transportation, which will enable the incumbent to travel to care facilities, home visits, and multiple hospitals.
