The Clinical Nurse Leader will oversee the planning, coordination, and assessment of daily clinical operations while acting as the primary resource for the nursing team. This role ensures that exceptional patient and family-centered care is delivered to every individual. The Clinical Nurse Leader will engage in collaboration, coordination, delegation, and supervision with the healthcare team to promote evidence-based practices and enhance patient care outcomes. They will also serve as a clinical reference for both clinical and administrative staff, supporting a collaborative approach to care across Great Lakes Bay Health Centers. Participation in Quality Improvement initiatives is also expected.

KEY RESPONSIBILITIES

Direct Patient Care Duties:

  1. Evaluate and adjust patient treatment plans based on their responses and conditions in partnership with other team members; oversee all facets of patient care.
  2. Conduct triage for patients via phone or in-person visits.
  3. Provide continuous education to patients and families regarding chronic illnesses, disease management, prevention, and medication adherence. Assess patient comprehension of their conditions and treatments, facilitate follow-up care, and ensure clarity of provider instructions.
  4. Communicate critical and abnormal test results to patients as directed by the healthcare provider.
  5. Manage Coumadin therapy; perform and address INR levels in the clinic.
  6. Review patient self-management tools such as blood pressure and A1C logs.
  7. Execute standard and specialized diagnostic tests and procedures independently, following nursing protocols or under provider supervision, including IV therapy, venipuncture, immunizations, EKGs, lab tests, medication administration, nebulizer treatments, visual exams, spirometry, and any other necessary testing.
  8. Assess medication requests and facilitate prescription refills in accordance with the established medication refill policy; dispense stock medications under provider guidance and according to organizational policies.
Patient Documentation and Charting Duties:

  1. Monitor, document, and report symptoms and changes in patients' conditions; record medical information and vital signs in the electronic health record (EHR). Maintain precise and comprehensive reports and records. Complete all necessary paperwork related to patient visits.
Nursing and Clinic Team Duties:

  1. Identify high-risk patients for referral to Care Management Nurses and/or other appropriate team members.
  2. Facilitate patient care activities and manage patient flow within the clinical environment.
  3. Consult and collaborate with healthcare team members to assess, plan, implement, and evaluate patient care strategies.
  4. Assist in the training and supervision of new nursing staff, nursing students, and other healthcare personnel.
JOB REQUIREMENTS
  1. Education: Graduate from an accredited nursing program; BSN preferred.
  2. Licensure: Registered Nurse with active licensure to practice in the State of Michigan. Basic Life Support certification required.
  3. Experience: Minimum of two years in acute medical/surgical nursing with demonstrated clinical, leadership, communication, critical thinking, and problem-solving skills.
  4. Physical Effort: Medium.
