Position Highlights:

  • Supports a designated provider team to enhance patient access and care delivery.
  • Standard working hours are Monday through Friday.

Role Responsibilities:

The individual in this role will deliver patient care aligned with established practice standards. This includes increasing patient access through both in-person and virtual consultations, while collaborating with team members to address patient needs effectively.

  • Collaboration: Work closely with assigned providers to ensure optimal patient care by:
    • Engaging in the healthcare team.
    • Facilitating communication among team members and other healthcare professionals.
    • Serving as a resource for team members.
    • Collaborating with the primary care team for program development.
    • Acting as a liaison with external agencies as necessary.
  • Clinical Care: Provide comprehensive clinical services to patients by:
    • Conducting thorough patient assessments.
    • Establishing diagnoses and treatment plans.
    • Ordering and interpreting medical diagnostics in conjunction with the attending physician or advanced practice clinician.
    • Prescribing medications, therapies, and medical equipment.
    • Delivering patient education for chronic illness management and wellness care.
    • Referring patients to specialty care when appropriate.
    • Consulting with physicians or experienced advanced practice clinicians when patient needs exceed the scope of practice.
  • Additional Duties: Perform other responsibilities as assigned.

Qualifications:

  • Completion of a Master's or Doctoral Nurse Practitioner or Physician Assistant program.
  • New graduates are welcome to apply.

Preferred Experience:

  • One year of experience in Primary Care as a Nurse Practitioner or Physician Assistant.

Licenses and Certifications:

  • Registered Nurse (for Nurse Practitioners) license in the states of Minnesota and/or Wisconsin.
  • Nurse Practitioner or Physician Assistant license in the states of Minnesota and/or Wisconsin.
  • DEA certification required.
  • BLS certification required within 180 days of hire.

Physical Requirements:
Medium Work: Ability to lift up to 32 lbs. occasionally and up to 25 lbs. frequently, in accordance with the Allina Safe Patient Moving Policy.
