Position Highlights:
- Role designed to enhance patient access and care by supporting a designated provider team.
- Work schedule: Monday to Friday.
Role Responsibilities:
The individual in this position will deliver patient care aligned with established practice standards. This includes providing direct patient care through both in-person and virtual consultations, as well as collaborating with other team members to address patient needs between visits.
- Collaboration: Work closely with assigned providers to ensure comprehensive care for patients, including:
- Engaging in healthcare team activities.
- Facilitating communication among team members and other healthcare professionals.
- Serving as a resource for team members.
- Contributing to program development alongside the primary care team.
- Acting as a liaison with external agencies as necessary.
- Clinical Care: Provide clinical services to patients, which involves:
- Conducting thorough patient assessments.
- Establishing diagnoses and treatment plans.
- Ordering and interpreting medical diagnostics in collaboration with the attending physician or APC.
- Prescribing medications, therapies, and medical equipment.
- Delivering patient education for chronic illness management and wellness.
- Referring patients to specialty care when appropriate.
- Consulting with physicians or experienced APCs for complex cases.
- Additional Duties: Perform other responsibilities as assigned.
Qualifications:
- Completion of a Master's or PhD program in Nurse Practitioner or Physician Assistant.
- New graduates are encouraged to apply.
Preferred Experience:
- At least one year of experience in Primary Care as a Nurse Practitioner or Physician Assistant.
Licenses and Certifications:
- Registered Nurse (for NP) license in the states of MN and/or WI.
- NP or PA license in the states of MN and/or WI.
- DEA certification required.
- BLS certification required within 180 days of hire.
Physical Requirements:
Medium Work: Lifting up to 32 lbs. occasionally and up to 25 lbs. frequently, in accordance with Allina's Safe Patient Moving Policy.
